- 3 jaar werkervaring als Wmo-consulent binnen een gemeente
- Ervaring met het HHM-normenkader 2.0 voor begeleiding
- Ervaring met Suite (Centric) is een pré

- In gesprek gaan met inwoners over hun hulpvragen
- Keukentafelgesprekken voeren en situaties zorgvuldig in kaart brengen
- Beoordelen wat iemand zelf kan en wat het netwerk kan betekenen
- Ondersteuningsplannen opstellen op basis van afwegingen tussen voorzieningen en maatwerk
- Schakelen met inwoners, zorgaanbieders en collegas
- Snel tot de kern van een vraagstuk komen
- Overzicht houden bij complexe casuïstiek en werkbare oplossingen vinden
- Zorgen dat inwoners de ondersteuning krijgen die bij hun situatie past
Description
Wij bieden een mooie opdracht binnen een betrokken gemeentelijke organisatie waar jouw werk direct impact heeft. De opdracht is voor 28 uur per week voor de duur van 7 maanden, met optie tot verlenging. Inschaling conform schaal 9 (cao gemeenten) en start per 1 mei 2026.
