Must:
- Geregistreerd huisarts, bij voorkeur kaderarts palliatieve zorg of bereid dit te worden
- Kennis van landelijke zorgstandaarden en palliatieve zorg-richtlijnen
- Sterke communicatieve vaardigheden en visie op zorginhoud
- Teamspeler met overtuigingskracht en enthousiasme
- Actueel verklaring omtrent gedrag (VOG) vereist

Responsibilities
- Advies en aanspreekpunt voor palliatieve zorgvragen binnen de organisatie
- Inhoudelijk bijdragen aan kwaliteit en innovatie van palliatieve zorg in de eerste lijn
- Proactief en reactief contacten onderhouden met huisartsen en hun personeel over palliatieve (huisartsen) zorg
- Verzorgen van (na)scholingen en advisering en coaching voor huisartsen en medewerkers
- Vertegenwoordiging in overleggen met regionale ketenpartners, zorgverzekeraars, ziekenhuizen en farmacie
- Meedenken over transmurale afspraken
Description
Wij zijn op zoek naar een Kaderarts palliatieve zorg (in opleiding) voor de regio Zutphen, waar we een prettige werksfeer bieden met ruimte voor eigen initiatief. Wij bieden een volledige vergoeding van de kaderopleiding, arbeidsvoorwaarden conform de CAO Huisartsen in loondienst en een dienstverband van 2 jaar met uitzicht op verlenging. De startdatum is in overleg, met het doel de vacature in twee jaar volledig te hebben ingevuld.
